Noble County Health Fair

The public isinvited to attend a health fair Sept. 11 from 9 a.m. to 1:30 p.m. at the Parkview Noble Hospitalcampus, 401 Sawyer Road,in Kendallville.

The eventwill feature more than 20 booths from departments within the hospital andseveral community agencies, including CANI. Reduced cost blood tests will alsobe available. In addition to the health fair, there will be a 5k run/walk and avariety of children’s activities.

CANIwill provide information on the state-funded health insurance plans availableand will assit with the application process. The Healthy Indiana Plan (HIP) isintended for adults ages 19-64 who do not have access to health insurance. HoosierHealthwise is the free or low cost state-sponsored health plan for low-incomefamilies, children up to age 19, and pregnant women.

Seebelow for information on the needed documentation for the application process:

 

Ø      Social Security Numbers-foreveryone living in the household

 

Ø     Identity-for each applicant and parent or guardian

Validdrivers license, State Identification Card, student ID, Military ID. If youhave someone acting on your behalf, that person will need to provide proof ofhis or her identity also. If child is under the age of 16 the IdentityAffidavit for Children Under Age 16 must be submitted.

 

Ø     US Citizenship-for each applicant

Legalbirth certificate, Certificate of Naturalization, Certificate of Citizenship orU.S.passport if it was issued without restrictions.

 

Ø     Money received by applicant, spouse, and dependent children in thehome

 

1. Wages: Pay stubs, paychecks, or statement from employer for the past 30days. You will need 60 days of pay stubs if you want coverage to go retroactiveback 60 days. Hoosier Healthwise can go retroactive back 90 days; therefore youwill need 90 days of pay stubs.

 

2. Self-employment: Last year’s signed tax return or personally keptself-employment records for the past 90 days.

 

3. Child Support, Social Security, VA, SSI, Workers Compensation,disability, sick pay, unemployment, or other benefits: Court order, award letter, orother proof of payment from the source of income.

 

4. Loans, gifts or contributions: Promissory note; loan agreement;or statement from the person providing the money that includes the person’sname, address, phone number, signature and date.

 

5. Financial aid award letter for the current year (if applicant is currently incollege and receiving any grant or financial aid).

 

6. Wage verification from unemployment office (if currently unemployed and notreceiving an income).

 

Ø      Guardianship or Power ofAttorney

Ifsomeone has legal authority to act on your behalf, provide a copy of the Powerof Attorney, Guardianship Order, Court Order, or similar documents.

 

Ø     Immigration status

A copy of your alien registration card, permanent resident cardor other documentation from the Bureau for Citizenship and Immigration Services(formerly the INS).

 

Ø     Documentation of pregnancy from a medical provider
